11/18/2023 EnchantaVaughn Approved QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Ms. Broadnax verbalized to QMHPA that her morning was going okay. QMHPA engaged her in a discussion to determine if she had handled the situation that she discussed with QMHPA during the last session. Ms. Broadnax verbalized to QMHPA that she did not because she got angry about the situation all over again and she wasnât ready to address the problem. QMHPA informed Ms. Broadnax that this session was going to focus on reducing her depression and anxiety symptoms. Ms. Broadnax did not present any symptoms of SI or HI at the time of the session being conducted. QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings and emotions. QMHPA began the session by inquiring about Ms. Broadnaxâs medication management. QMHPA educated Ms. Broadnax on depression/anxiety. QMHPA processed with Ms. Broadnax how social anxiety could cause deeper depression symptoms. QMHPA assisted Ms. Broadnax with interacting with other throughout the community. QMHPA guided Ms. Broadnax through appropriate communication techniques. QMHPA engaged Ms. Broadnax in skills building activity. QMHPA explained to Ms. Broadnax that the purpose of this activity was for QMHPA to demonstrate Ms. Broadnax proper terminology to greet others and start a conversation. QMHPA explained to Ms. Broadnax when she converses with others try to look them their eyes and the importance of practicing good posture. QMHPA engaged Ms. Broadnax in a skills building activity on positive and negative self-esteem. QMHPA explained to Ms. Broadnax that the purpose of this activity was to prepare her for any challenges that she is faced with when resolving conflict and other situations that may cause depression. Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Broadnax stated to QMHPA that she did not adhere to her medication for today because she had a headache and that she had took some other medication and she didnât know how it was going to react to her body. Ms. Broadnax said she becomes depressed because there is so much that she has to endure and she at times feels as though she is going it alone. Ms. Broadnax expressed to QMHPA that although she has a boyfriend he doesnât assist much. Ms. Broadnax verbalized to QMHPA that she becomes depressed at night time because there is so much going on in the community. Ms. Broadnax stated she just lay down or pace when experiencing anxiety. Ms. Broadnax was able to comply with appropriate communication techniques. Ms. Broadnax said she is happy with herself when he is being good for positive self-talk.
